ELIGIBILITY UPDATES

UNDER NEW DIAA AND DEPARTMENT OF EDUCATION GUIDELINES
A GUARDIAN MUST COMPLETE THE FOLLOWING STEPS IN RANKONE FOR YOUR STUDENT ATHLETE TO BE ELIGIBLE TO PLAY IN ANY SPORT

DOVER HIGH SCHOOL ATHLETICS WILL NOT ACCEPT ANY PHYSICAL OR EMAIL FORM OF A STUDENT-ATHLETES PHYSICAL. THE ONLY ACCEPTED FORM WILL BE THROUGH RANKONE.

Parent Instructions for Completing Medical Forms

1. Set up your RankOne account by navigating to capitalschooldistrict.rankone.com
2. Create your account. 
   • Follow the instructions here https://education.delaware.gov/diaa/resources/rankone/ to create your parent account. 
3. Add your student(s). 
   • Follow the instructions here https://education.delaware.gov/diaa/resources/rankone/ under How to Add a Student to your Parent Account.
4. Complete the required forms in RankOne.
   • Follow the instructions here https://education.delaware.gov/diaa/resources/rankone/ under How to Complete Required Forms.
   • Required forms in RankOne for NEW and RETURNING students:
     • 1. DIAA Eligibility Form
       2. DIAA Acknowledgement Form
       3. Pre-Participation Consent Form 
       4. Emergency Contact Card
       5. Medical Card Upload Form – Upload a copy of the Student Athlete Medical Card, completed and signed by a physician. THIS IS PAGE 5 OF YOUR 26-27 DIAA PHYSICAL.

ALL 5 FORMS MUST BE SUBMITTED IN ORDER TO BE ELIGIBLE FOR THE 2026-2027 SCHOOL YEAR!!!


REMINDERS:

• All athletic registration must be done in Rank One. No documents for athletics will be accepted at the school.
• To be eligible to participate in interscholastic athletics for the 2026-2027 school year, all student-athletes need a DIAA PPE (preparticipation physical exam form) completed by their physician or qualified healthcare provider after April 1, 2026.
• DIAA has created a new PPE form.  This will be the only form accepted for the 2026-2027 school year. The new form will be posted on the DIAA and school athletic websites.  Please see the DIAA MEDICAL FORMS PAGE page (click link) for more information.

*DIAA Physical form must be completed by parent/guardian prior to physical activity.
